Incident Report Form

Step 1:

  1. Navigate to the Report List from the main menu.
  2. Click the Create Incident Report button. A new window will open.

  3. Type in the Incident Name.

  4. Choose the Start Date from the calendar. 

  5. Select the Type of Call from the drop-down. 

  6. Choose the AOR from the menu. AORs can be updated by clicking the Add New button

Step 2:

  1. Select Yes or No from the box labeled Person Calling Is In System? 

  2. Type in the Name of Person Calling

  3. Enter the Call Back Number

  4. Provide a Brief Synopsis of Incident

  5. Select Yes or No from the box labeled Did the Facility Muster? 

  6. Choose Yes or No in the box labeled Did the Facility report to Abandon Stations? 

  7. Pick an Incident Type from the drop-down.

  8. Select a Relevant Teams Identified from the menu. 

  9. Fill in the Resources Activated description. 

Step 3: 

  1. Click the Incident After Action Report file upload. 

  2. Choose a file from your device. 

  3. Repeat the steps as needed.

  4. Click the Exercise After Action Report file upload. 

  5. Choose a file from your device. 

  6. Repeat the steps as required. 

  7. Click the Send button. 

Make sure all required fields are filled before submitting the form.

View Report 

Click View Report to see full report details and generate an ICS form. Once Generate ICS Forms is selected, members will receive an alert. Incidents are escalated to duty members when this form is completed. 

To generate an ICS form, follow these steps: 

  1. Find the Incident Name

  2. Click the associated more options icon (ellipses).

  3. Press the View Report button

  4. Select a Report Status from the drop-down menu. 

  5. Press the Generate ICS Forms button.

  6. Click OK to confirm form generation. 

Edit Report 

  1. Find the Incident Name

  2. Click the associated more options icon (ellipses).

  3. Select the Edit Report button

  4. A new page will open.

  5. Update the report details. 

  6. Click the Save button

Delete Report 

  1. Locate the Incident Name

  2. Click the associated more options icon (ellipses).

  3. Choose the Delete Report button.

  4. Press the Delete button to confirm. 

Warning: Once a report is deleted, it cannot be recovered.
